Using the patient portal is easy and secure and gives you access to your health care when it’s convenient for you, 白天或晚上. A patient portal offers several benefits. You can schedule an appointment when it’s convenient for you and the phones may be closed, request prescriptions or directly communicate with your health care team. You can also view your health records, access 访问总结 and other documentation.

We will send a parent/guardian invitation to view and manage your child’s account. Check the email we have on file for you, if no email is on file for your child simply update the record with any receptionist or staff member.
The parent/guardian needs to enroll in the portal first, then create an account for each child. All accounts can be linked to the parent account for easy management.
More than one parent can have access to their child’s information. For example, divorced parents will have their own access to view their child’s portal account.

Log in to your patient portal account and click the “Health Forms” tab to view and complete your forms.
